      Introduction

      Lung cancer is the leading cause of cancer death worldwide with majority of new cases (51%) now occurring in Asia-Pacific with increasing incidence. Valuable real-world evidence on the evolving landscape of molecular testing, treatment options, and clinical outcomes of Asia-Pacific patients is limited. The EXPLORE-LC program (EXtensible PLatform for Observational REsearch in Lung Cancer) aims to build a network of treating clinicians and combine high-quality real-world data sources, or ‘RWDS’, to carry out real world research.

      Methods

      The EXPLORE-LC program will be a coordinated meta-analysis of lung cancer databases across APAC creating a novel multinational and multiple-source evidence network to support lung cancer research. The program will feature a multi-time-point retrospective cohort design, initially focused on non-small cell lung cancer, with the primary objective of understanding real-world outcomes stratified by histological stage and biomarker status. Standardized analysis will be implemented to describe patient characteristics and treatment patterns from diagnosis until lost to follow-up or death. RWDS and physicians will be identified from existing IQVIA partnerships and will be further selected based on case volume, generalizability of the case population and completeness of available data.

      Using a common data model approach across RWDS, specific lung cancer variables will be extracted into a locally-maintained EXPLORE-LC data file. Where required, key variables may be enhanced through additional linkages or text-mining activities to build on existing data and generate the study-specific dataset. Standardized analyses will then be run locally at each RWDS to generate source-specific summary statistics, which will be pooled through national and regional-level meta-analyses by IQVIA’s central analytics team.

      Variables analyzed will include histological sub-type, biomarker status, initial treatment, line of therapy and treatment sequencing, as well overall survival, treatment duration and time to next treatment.

      Results

      Pilot EXPLORE-LC analysis is being conducted at leading partner institutions and registries; Asan Medical Center, Samsung Medical Center and Severance Hospital, Yonsei University Health System in South Korea and the Victorian Lung Cancer Registry (VLCR) in Australia. Results on approximately 35,000 NSCLC cases will be reported by Q1 2021. Additional RWDS in China, Taiwan and other APAC countries will be included between 2021 and 2022 with each participating RWDS nominating a lead investigator to join the EXPLORE-LC steering committee to shape future research.

      Conclusion

      The EXPLORE-LC framework will create the first Asia-Pacific wide real world research platform for lung cancer, enabling valuable evidence generation on patient management trends and real-world outcomes that have historically been visible only at individual data sources. The use of a common data model approach along with meta-analyses works within patient data privacy and regulatory requirements which have limited large scale pooling of clinical and genomic data. Lastly, EXPLORE-LC will provide a unique platform for multi-source collaboration and further analyses into specific lung cancer sub-populations with Asia-Pacific, helping to uncover unmet needs and further improve the management of this important disease in the region.
