      Background

      Lung cancer is one of the most common cancers in the world in terms of both incidence and mortality. In patients with advanced lung cancer, the progressive decline in nutritional and functional status - particularly unintended weight loss - has been strongly associated with reduced physical functioning, poor tolerance to anticancer therapy, decreased quality of life, and poor prognosis and survival.

      The aim of the present study was to assess and characterize the nutritional status of patients with lung cancer at the moment of diagnosis.

      A retrospective study was conducted on 2655 patients with lung cancer at São João University Hospital, in Porto, between 1976 and 2016. From the total number of patients, 68,8% (n=1827) had the nutritional assessment for the nutritional characterization, therefore the sample of the present study was 1827 patients. The characterization of the nutritional status was based on the WHO classification of Body Mass Index (BMI) by Quetelet. The cut-off points of weight loss used were: <5%, 5-10%, ≥10%. The lung cancer histology was grouped by non-small cell lung cancer (NSCLC) and small cell lung cancer (SCLC). The sample was split by gender (female, male) and age groups (≤45, 46-69 and ≥70 years old). The statistical analysis was calculated using the SPSS® programme.

      From the total sample (n=1827), 85,1% were male and 14,9% female. The median age was 63 years (min= 19 years old, max= 92 years old).

      According to the WHO BMI classification, 8,8% of the eligible patients were underweight, 55,4% had normal BMI, 23,1% were overweight and 5% were obese. 67,9% of the patients had NSCLC and 19% had SCLC.

      From the total sample with NSCLC, 3,1% presented <5% of weight loss, 15% had 5-10%, and 31,3% had ≥10% of weight loss. Furthermore, patients with NSCLC presented more underweight (9,8%) than those with SCLC. A high percentage of underweight (92,5%) was found in older patients (≥46 years old) and, simultaneously, a significantly percentage of weight loss (>10% of weight loss = 82,1%) was also shown.

      The high prevalence of weight loss supports the current literature. The progressive decline in nutritional status in the patient with advanced lung cancer demonstrates that a precocious assessment of the nutritional status and follow-up can be an effective tool.
