      Background

      In last decades several changes have been observed in lung cancer incidence, morphology, diagnostic and treatment options. However, only a few studies have focused on these changes among surgically treated patients.

      The aim of the current study was to analyse changes in lung cancer surgical treatment, comparing surgically treated patients with all lung cancer patients.

      Data on all surgically treated lung cancer patients in Estonia from January 2000 to December 2015 were obtained from hospitals’ medical records. Data on all patients diagnosed with lung cancer in the same period were obtained from the Estonian Cancer Registry. For analysis the study period was divided into 4-year time periods.

      During the study period 2075 patients (1582 male, 493 female) with mean age 65 (range 10−89) years had been treated surgically in Estonia. The number of patients diagnosed with lung cancer did not change in the 16 years, but proportion of patients treated surgically increased from 13% to 19%. Among surgically treated patients the proportion of women increased from 18% to 31%; while this change among all lung cancer patients was from 20% to 27%. The proportion of younger patients (<60 years) among those treated surgically decreased from 34% to 28%, while the proportion of patients in the oldest age category (>79 years) quadrupled from 2% to 8%; the respective changes among all lung cancer patients were less pronounced. Adenocarcinoma became the most frequent morphological type of surgically treated lung cancer (replacing squamous cell carcinoma), with an increase from 23% to 43%. The proportion of adenocarcinoma was constantly higher among female (compared to male) surgical patients, but the proportion of adenocarcinoma increased among males in all the age groups except >79 years, yet remained stable among females.

      Lobectomy has been the most common type of lung resection, with an increase from 53% to 76% over the study years. The proportion of pneumonectomy decreased from 28% to 8%. VATS lobectomy was introduced in 2006 and has been applied in an increasing number of cases. In 2015, the VATS approach accounted for 56% of lung resections.

      Although the number lung cancer cases diagnosed in Estonia each year has not changed since 2000, the proportion of patients treated surgically has increased. Among surgical patients, the proportion of women and elderly has increased. Adenocarcinoma has become the most common morphological type of lung cancer. Most importantly, thoracotomy has been gradually replaced by VATS.
